Outline | CoQ10 is a coenzyme widely present in the living world as a liposoluble element of respiratory chain, and acts as an electron carrier in the electron transport system in mitochondria. As a pharmaceutical product, it is used for treatment and study of angina. The heart has automaticity due to the conducting system, and is also influenced by autonomic nerves. The sympathetic nerve acts in an exciting manner, and the parasympathetic nerve in a suppressive manner. Physiological action of cardiotonic glycoside It directly and selectively acts on the heart muscle and enhances the myocardial contractility. - It acts on the process of excitation-contraction coupling, enhances the myocardial contractility, and increases the volume of blood pumped out. This effect occurs by inhibiting Na+, K+-ATPase of the cardiac sarcolemma and by inhibiting the Na+ pump. - It decreases heart rate. This event is associated with the following two factors: 1. Factor via the vagal nerve (negative chronotropic effect ), and 2. Direct effect on the heart to delay conduction in the atrioventricular node (negative dromotropic action). - The threshold value for stimuli from outside of the atrium and chamber increases by administration of digitalis, and sensitivity decreases. On the other hand, automaticity increases. Therapeutic agent for antianginal effect, improvement of congestive heart failure, and treatment of cerebrovascular accident. |
